State of Play Review on Haemorrhagic Stroke Basic science of intracerebral haemorrhage

چکیده
Overview Intracerebral haemorrhage (ICH) causes brain injury in 3 major ways. Firstly there is the mass effect of the initial bleed, haematoma expansion and oedema formation, which increases intracranial pressure causing distortion of brain structures, and mechanical damage to cells. Secondly, there is neuroinflammation that results in secondary brain damage following ICH, through glial activation, cytokine production and disruption of the blood brain barrier (BBB). Finally, rapid dissolution of clotted blood causes the release of breakdown products, such as haemoglobin, haem and iron, which are toxic, cause oxidative stress through the production of free radicals and brain inflammation.

متن کاملAcute haemorrhagic stroke.
OBJECTIVE To review the management and some of the recent advances in acute haemorrhagic stroke. DATA SOURCES Articles and published reviews on acute haemorrhagic stroke. SUMMARY OF REVIEW Hypertensive intracerebral haemorrhage or subarachnoid haemorrhage (SAH) from a ruptured intracranial saccular aneurysm are the commonest causes for an acute haemorrhagic stroke. Both lesions are often cl...
متن کاملAggressive statin treatment, very low serum cholesterol levels and haemorrhagic stroke: is there an association?
PURPOSE OF REVIEW To assess the potential association between haemorrhagic stroke and achieving very low serum cholesterol levels with aggressive statin treatment. RECENT FINDINGS The Stroke Prevention by Aggressive Reduction in Cholesterol Levels (SPARCL) study showed a reduction in both fatal and nonfatal stroke but an increase in the risk for haemorrhagic stroke during high-dose atorvastat...
متن کاملIntracerebral haemorrhage after thrombolysis for acute ischaemic stroke: an update.
Intracerebral haemorrhage (ICH) still represents the most feared complication of thrombolysis. Our aim was to review the literature regarding clinical, biological and imaging predictors of ICH following thrombolysis for acute ischaemic stroke. Relevant studies were identified through a search in Pubmed, using the following key words: "intracerebral", "haemorrhage", "stroke" and "thrombolytic". ...
متن کاملThe impact of delays in computed tomography of the brain on the accuracy of diagnosis and subsequent management in patients with minor stroke.
OBJECTIVES To determine the proportion of haemorrhagic strokes misdiagnosed as infarcts on computed tomography (CT) in patients with mild stroke, and the implications for health care. METHODS Patients with mild stroke presenting as inpatients or outpatients four or more days after stroke to our stroke service (catchment population 500 000) were recruited prospectively. They underwent detailed...
